Mini-Census (Day 11)
Day 12 - Week 12 Non-Medical Withdrawal AEM Form will be available after mini census

Provide supporting documentation

Signatures: Instructor, Department Chair, Dean/Designee

"W" grade will remain on academic record (transcript)

&

Counts toward your 18-unit undergraduate withdrawal limit

Day 12 - Week 15 Medical Withdrawal AEM Form will be available after mini census
